1. Who we are and what this policy covers

This website, innova-sleep.com (the “Site”), is operated by Innova Sleep Institute, LLC (“Innova Sleep,” “we,” “us”), 149 Scranton Connector, Brunswick, Georgia 31525. Innova Sleep Institute is the oral appliance sleep apnea service affiliated with Innova Dental Implant Institute.

This policy explains what information we collect when you visit the Site, how we use it, who we share it with, and the choices you have. It applies to the Site only. It does not apply to information in your patient record. How we use and protect your protected health information as a patient is described in our Notice of Privacy Practices under HIPAA, which you receive when you become a patient and which is available on request.

The Site is for general information. Using it does not create a doctor-patient relationship. Please do not send detailed medical information through the Site. If you need to share health information with us, call us at (912) 266-8100 or discuss it at your visit.

2. Information you give us

You may choose to give us information by using the insurance checker, filling in a form, requesting a consultation, scheduling online, signing up for text messages, or contacting us. This can include your name, phone number, email address, how you prefer to be contacted, and your message.

Insurance checker. The “Check If You’re Covered” tool asks for your insurance carrier and plan type so we can tell you whether oral appliance therapy for sleep apnea may be covered and follow up with you. We use your answers only for that purpose. They are delivered to our practice by email and kept in our own systems. They are not sent to any analytics or advertising service. Submitting the checker is optional, and you can call us instead.

Online scheduling is handled through Acuity Scheduling. When you book, the information you enter is collected by that service on our behalf under a written agreement that requires it to protect your information.

3. Information collected automatically

When you visit the Site, our web server and the services below may automatically record technical information: your IP address, browser type and settings, device type, the pages you view, the date and time of your visit, the website that referred you, and how you interact with pages. Some of this information is collected through cookies and similar technologies.

We use a consent banner. Analytics and advertising-measurement cookies are not set, and no analytics or advertising request is sent from your browser, until you choose “Accept” on the banner. If you choose “Reject,” only cookies that are strictly necessary for the Site to work are used. You can change your choice at any time using the “Cookie settings” link in the footer of every page.

4. How we use information

We use the information described above to answer your insurance question and respond to your inquiry; to schedule you; to send you text messages you have asked for; to operate, secure and improve the Site; to understand how visitors use the Site and whether our advertising works; to comply with law; and to protect our rights and the safety of our patients and staff.

We do not sell your personal information. We do not use information collected on the Site, including your insurance answers, to build advertising audiences or to show you targeted advertising, and we do not share it with advertising platforms for those purposes.

5. Who we share information with

We share information with service providers that help us operate the Site and our practice, such as website hosting and development, online scheduling, patient communication and customer-relationship software, text-messaging delivery, and the analytics and measurement services listed in Section 3. These providers may use the information only to provide their services to us. Where a provider handles information that could identify you as a patient, we have a written agreement with that provider that requires it to protect the information as the law requires.

If you become a patient, we work with a sleep medicine partner for sleep testing, diagnosis and insurance billing. Information shared with them is protected health information governed by HIPAA and our Notice of Privacy Practices, not by this website policy.

We may also disclose information when required by law, subpoena or court order; to protect the rights, property or safety of Innova Sleep, our patients, our staff or others; or in connection with a sale, merger or reorganization of our practice.

Text-messaging opt-in data and consent are not shared with any third party for their own marketing purposes.

6. Text messages (SMS)

If you give us your mobile number and agree to receive text messages, we may text you about appointments, your inquiry, and our services. Message frequency varies. Message and data rates may apply. You can opt out at any time by replying STOP to any message, and you can get help by replying HELP, emailing hello@innovaimplants.com, or calling (912) 266-8100. Consent to receive texts is not a condition of receiving care.

7. Your choices and rights

Cookies and analytics. Use the consent banner, or the “Cookie settings” link in the footer, to accept or reject analytics and measurement cookies at any time. You can also block or delete cookies in your browser settings. If your browser sends a Global Privacy Control signal, our consent tool treats it as a request to opt out, and non-essential cookies stay off unless you choose to allow them.

Marketing communications. You can opt out of texts as described in Section 6 and ask us to stop other communications by contacting us using the details in Section 12.

Access, correction and deletion. You may ask us what personal information we hold about you from your use of the Site, ask us to correct it, or ask us to delete it. We will respond within the time required by applicable law and may need to verify your identity first. Some information cannot be deleted where we are required to keep it, for example information that has become part of your patient record, which is governed by HIPAA and our Notice of Privacy Practices. We will not treat you differently for exercising these rights.

Depending on where you live, you may have additional rights under your state’s privacy law. You can exercise any of these rights by contacting us as described in Section 12.

8. Security

We use reasonable administrative, technical and physical safeguards to protect information collected through the Site, and we limit access to it to people who need it to do their jobs. No website or internet transmission is completely secure, so please do not send sensitive medical information through the Site.

9. How long we keep information

We keep information you submit through the Site, including insurance checker answers, for as long as needed to respond to you and for our records, and longer where the law requires it or where it becomes part of your patient record. Analytics data is retained by Google Analytics for 14 months and is then deleted automatically.

The Site is not directed to children under 13, and we do not knowingly collect personal information from children through the Site. Parents or guardians may contact us about a minor’s care directly.
